{"id":64589,"date":"2015-02-19T08:00:52","date_gmt":"2015-02-19T14:00:52","guid":{"rendered":"http:\/\/csinewsnow.com\/?p=64589"},"modified":"2015-02-19T14:48:31","modified_gmt":"2015-02-19T20:48:31","slug":"community-health-assessment-survey-available","status":"publish","type":"post","link":"http:\/\/csinewsnow.com\/?p=64589","title":{"rendered":"Community Health Assessment Survey available"},"content":{"rendered":"<p>&nbsp;<\/p>\n<p><a href=\"http:\/\/csinewsnow.com\/wp-content\/uploads\/2013\/10\/medicalgeneric.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignleft size-full wp-image-29907\" alt=\"medicalgeneric\" src=\"http:\/\/csinewsnow.com\/wp-content\/uploads\/2013\/10\/medicalgeneric.jpg\" width=\"170\" height=\"113\" \/><\/a><\/p>\n<p>Jamestown (CSi) A Community Health Assessment Survey, is available on line, to assess the area concerning health issues and concerns.<\/p>\n<p>The survey is a partnership between Jamestown Regional Medical Center, Central Valley Health and the Community Health Partners.<\/p>\n<p>The partnership is hoping that at least 400 surveys completed from community members.<\/p>\n<p>An online survey can still \u00a0be accessed at <a href=\"http:\/\/www.tinyurl.com\/jamestown-nd\">www.tinyurl.com\/jamestown-nd<\/a><\/p>\n<p>A paper survey option is available at Central Valley Health and Jamestown Regional Medical Center.<\/p>\n<p>The survey deadline is March 1<sup>st<\/sup> \u2013 an extension from February 20<sup>th<\/sup>.<\/p>\n<p>The focus of this effort is to:<\/p>\n<p>\u00b7 Learn the good things in the community and the community\u2019s concerns<\/p>\n<p>\u00b7 Understand perceptions and attitudes about the health of the community and hear suggestions for improvements<\/p>\n<p>\u00b7 Learn more about how local health services are used by you and other residents<\/p>\n<p>From this effort, the community health partners will collaborate and compare the results to the assessment completed 3 years ago.<\/p>\n<p>Officials are confident that with these results, community health teams can better set goals based on the priority areas determined in\u00a0the area.<\/p>\n<p>&nbsp;<\/p>\n<p>Jamestown (CSi)\u00a0 The Community Health Partnership committee is conducting a community health assessment survey.<\/p>\n<p>Responses will be used to prioritize areas where improvement is needed, as well as recognize where the committee is doing well.<\/p>\n<p>In an E-Mail the information says that he brief survey will help in developing the most accurate perspective on what priority improvements are most needed.<\/p>\n<p>The committee\u2019s vision is to be the healthiest community in which to live, learn, work, and play.<\/p>\n<p><span style=\"font-size: large;\"><a href=\"https:\/\/und.qualtrics.com\/jfe\/form\/SV_1L0FBXQnbg8b1sh\"><i><span style=\"font-size: large;\">Click here to complete the brief survey.<\/span><\/i><\/a><i><\/i><\/span><i><i><\/i><\/i><\/p>\n<p>The community health assessment survey is conducted every three years.<\/p>\n<p>Partners will use the assessment process to formulate a community health improvement plan aimed at striving to provide effective, quality health services and an environment that enables community members to reach their full potential through assessment, leadership and partnerships.<\/p>\n<p>&nbsp;<\/p>\n<p>The results will be available at the end of June and released in great detail sharing specific improvement priorities for the community health partnership team.<\/p>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>&nbsp; Jamestown (CSi) A Community Health Assessment Survey, is available on line, to assess the area concerning health issues and concerns.
