Phyllis M. Schultz, 86, of Forman, ND, passed away Friday, January 16, 2026 at Four Seasons Healthcare – SNF.

Phyllis was born August 5, 1939 in Wisconsin. She lived in Fargo for some time before moving to Forman in 1999.

Cremation has taken place and no services are scheduled at this time.
